Intra-abdominal vagal nerve blockade and malabsortive devices for severe obesity.

Original Title: Bloqueo vagal intrabdominal y dispositivos malabsortivos para el tratamiento de la obesidad grave
Authors' objectives: to assess the effectiveness and safety of malabsorptive devices and intra-abdominal vagal blockade in the treatment of severe obesity, in terms of reduced body weight, control and quality of life associated comorbidities was performed, and the frequency and severity of adverse events.
Authors' results and conclusions: In relation to the decrease in body weight, diabetes control, cardiovascular risk factors and quality of life, generally all patients had improvements without obtaining significant differences between the control and intervention group. Only one study found differences between groups at 6 months on the different parameters but at 12 months only for excess weigth loss (% EWL) and total percentage of weight loss remained the differences (level of evidence 1-). Adverse events were common (over 60 %), mainly mild and moderate gastrointestinal symptoms and pain had an important role. Satisphere™ device were numerous migrations, forcing it to suspend the study. In two clinical trials evaluated, they were rated as low quality (level of evidence 1-). In both cases, there was no masking and attrition bias was relevant. The main results, most of the work, were biased because they did not provide statistical significance. It couldn´t be concluded on ValenTx device as studies were case series only (no control group). Not all devices studied are an improvement for weight reduction and control of comorbidities for patients with severe obesity. Only the effect of diet and exercise recommendations generated weight loss and improved quality of life.
