Authors' recommendations: Scarce evidence of moderate methodological quality suggests that the use of intrathecal opioids in patients with cancer pain is associated to less toxicity because of the use of lower doses, with pain control similar to that of conventional medical treatment with opioids. Scarce evidence of low methodological quality suggests that with the use of intrathecal opioids greater pain reduction would be achieved than with medical treatment in patients with non-cancer pain. Both the clinical practice guidelines and the coverage policies consulted consider this treatment in patients with intractable pain or unacceptable toxicity who have failed with conventional alternatives and who have shown good response to the intrathecal opioid administration trial.
