Living donor liver transplant

Authors' objectives:

To assess the available evidence on the effectiveness of live-donor liver transplants (LDLT).

Authors' recommendations: LDLT should only be authorised in centres of excellence with longstanding experience in hepatectomies and LT (figures of more than 700 LT already carried out, or more than a given number of consecutive hepatectomies -50?- without no adverse results recorded, have been mooted), and only in patients on the waiting list with well-defined criteria that point to a long wait. LDLT should be looked upon as an innovative procedure, the performance of which should be limited only to experienced and highly-qualified centres. Although alternative forms of LT make it possible to increase the number of transplants available, increasing supply, it is time to address what is now an unavoidable need, i.e. managing demand favouring patients on the list who can get the most out of the transplant. It seems evident that not all patients present the same survival, but that there are predictive factors (underlying hepatic disease, co-morbidity, age, etc) that must be analysed carefully. Moreover, sooner than later this situation will need to be addressed by setting up mechanisms of transparency and consensus-based prioritisation that guarantee fairness of access to these complex and costly procedures.
