Magnetic resonance imaging with tractography for brain white matter diseases

Authors' objectives: To assess the available evidence on the efficacy, safety and coverage policy related aspects regarding the use of magnetic resonance imaging with tractography in patients with brain white matter disease.
Authors' recommendations: Low quality evidence shows that MRI-T could detect white matter abnormalities in dementia and neurological movement disorders, cerebrovascular ischemia, brain trauma, myelopathies and cerebral palsy and possibly be a better guide in planning and performing neurosurgery due to temporal epilepsy and high-grade gliomas. However, evaluations are required to allow determining its advantages compared to standard therapeutic and diagnostic methods. Although the clinical practice guidelines identified mention the potential role of MRI-T for white matter diseases, they also point out that further studies are needed before incorporating it as a standard tool.
