Authors' recommendations: Description of Technology: Scrambler Therapy (ST) (also referred to as Calmare Pain Therapy and transcutaneous electrical modulation pain reprocessing; Calmare Therapeutics Inc.) is a noninvasive, transdermal treatment for the symptomatic relief of chronic pain. Treatment is performed by applying electrodes corresponding to the dermatome on the skin just above and below the area of pain. The system is a multiprocessor with artificial neurons that generate synthetic action potentials. Algorithms take into account previous outputs and frequency, duration, and amplitude of modulation, and are used to assemble these signals to deliver patient-specific electrostimulation. The manufacturer suggests that the resultant artificial no-pain signals created by the electrostimulator block real-pain signals. The postulated mechanism for prolonged pain relief is a resetting of calcium channels or remodulation of the pain response. Patient Population: The ST device is indicated for use in the symptomatic relief of chronic, intractable pain, postsurgical and posttraumatic acute pain, symptomatic relief of acute pain, and symptomatic relief of postoperative pain. The Food and Drug Administration does not specify its use for cancer patients.
