Authors' recommendations: Description of Technology: The iStent is a 1-millimeter, titanium, heparin-coated, L-shaped implant. The stent is designed for permanent implantation in Schlemm’s canal, where it helps to open the trabecular meshwork and reduce intraocular pressure (IOP). The first-generation stent has a snorkel that projects into the anterior chamber after placement of the device, 3 retention arches that help to hold the stent in place, and a foot or tip that projects into the collecting system of the eye. The stent is implanted via an ab interno (corneal) incision following successful cataract surgery. It is inserted through the phacoemulsification incision under topical anesthesia. Advantages of the trabecular micro-bypass stent include minimally invasive insertion, a clear corneal incision that spares the conjunctiva and sclera, and a low risk for hypotony. Patient Population: The iStent is indicated for use in conjunction with cataract surgery (often phacoemulsification) for the reduction of IOP in adult patients with mild to moderate open-angle glaucoma currently treated with ocular hypotensive medication.
