Ischaemic heart disease: a pathway to prioritisation

Authors' objectives: In 2013, the NHC published Strategic Overview: Cardiovascular Disease in New Zealand which highlighted IHD as an area of high materiality in terms of health gain and expenditure. Following this publication, the NHC explored the drivers of IHD health loss and identified areas along the IHD pathway of care where there existed the potential for health improvements and reduced expenditure. This analysis formed the basis for discussion amongst a multi-disciplinary cardiovascular working group (CWG). The CWG comprises sector-nominated representatives from across the field of cardiovascular care and met in Auckland in early 2014 to discuss where it felt areas of need existed on the IHD spectrum of care.
Authors' recommendations: Following that meeting, the NHC is now publishing Ischaemic Heart Disease (IHD): A Pathway to Prioritisation which recommends five areas for assessment within IHD: expanding cardiac rehabilitation services, improved management of low and intermediate risk chest pain pathways, avoiding unnecessary interventions, elective angioplasty with same day discharge and early extubation/discharge of low risk patients for coronary artery bypass grafting (CABG).
