Authors' objectives: The Swedish Government has given SBU the instruction to systematically evaluate knowledge of methods that might reduce injuries and complications of women giving with with focus on pelvic floor ruptures and sphincter injuries. The following issues will be highlighted: What risk factors exist for stretch marks and sphincter injuries (evidence for instrumentation, induction, large children, genital mutilation, external pressure, shearing, etc) Can the damage be prevented? Is it possible to reduce complications of sphincter injuries (pain, incontinence) through careful diagnosis of the injury? Diagnostics of resulting injuries with focus on endoanal ultrasound. All questions will also be highlighted from a health economic and ethical perspective.
