[Intra-articular hyaluronic acid (viscosupplementation) in osteoarthritis]

Authors' objectives: To assess the available evidence on the efficacy, safety and coverage policy related aspects regarding the use of intra-articular hyaluronic acid (viscosupplementation) in patients with osteoarthritis. Osteoarthritis (OA) or ostearthrosis is a frequent joint disease characterized by joint degeneration induced by the interaction of several factors. It is estimated that 1.5 million individuals in Argentina suffer from knee OA, and 900 thousand, from hip OA. Their clinical manifestations include pain, function loss and disability. Treatment is aimed at reducing symptoms, minimizing disability and limiting progression through both pharmacological and non-pharmacological measures (systemic or local). As regards local pharmacological treatment, intra-articular corticoids have widely been used and it is proposed that hyaluronic acid might have similar efficacy, but with a more sustained effect in time. Hyaluronic acid (HA) is a biological polysaccharide which distributes in the connective tissue. In an arthrosic joint, its synovial concentration is decreased; intra-articular HA treatment is known as viscosupplementation.
Authors' recommendations: The evidence found on the use of hyaluronic acid in knee osteoarthritis is of low quality; it did not show to be better than intra-articular corticoids or diclofenac. Evidence of low quality has not reported benefits in patients with hip, shoulder, ankle osteoarthritis or any other joint. The clinical practice guidelines do not recommend its use although some US private coverage policies consider it a potential therapeutic alternative only for specific cases of knee osteoarthritis.
