[Insulin infusion pumps for diabetic patients]

Authors' objectives: To assess the available evidence on the efficacy, safety and coverage related aspects regarding the use of IP in patients with diabetes.
Authors' recommendations: In general, moderate quality evidence could not demonstrate that the use of insulin pumps may improve glucose control or reduce hypoglycemia episodes compared with multiple dose treatments in patients with type I or II diabetes mellitus in children, adults and pregnant women. When comparing the different types of pumps, some studies showed that those wiht have glucose monitoring sensors improve glycosylated hemoglobin control, but the number of hypoglycemia episodes do not decrease. The automatic discontinuation pumps might reduce severe and nocturnal hypoglycemia episodes. In spite of the fact the evidence could not clarify the clinical relevance of their use; insulin pumps are usually recommended by CPGs and covered by health sponsors for specific diabetic patients with difficulties in achieving an adequate control. Choosing the type of pump would depend on the kind of problem the patient has (poor glucose and glycosylated hemoglobin control and/or severe hypoglycemia episodes).
