Authors' objectives:
To assess the available evidence on the efficacy, safety and coverage related aspects regarding the use of intrastromal rings in patients with keratoconus.
Authors' recommendations:
Evidence with low quality methodological evidence shows that intrastromal rings may be useful for patients with keratoconus, and a possible alternative to corneal transplantation. In the short term, statistically and clinically significant improvements were observed in corneal topography, refraction and visual acuity.
For combination treatment with intraocular lenses or with collagen crosslinking, there is little evidence supporting their use or defining the adequate combination.
Evidence comparing with other alternatives such as corneal transplantation or UV-CXL is still scarce.
The Clinical practice guidelines in general do not recommend intrastromal rings. No consensus was found in the analyzed coverage policies; they are not included in the Mandatory Medical Plan (PMO) or in the list of technologies that may be reimbursed through the Unique Reimbursement System of the Healthcare Service Superintendence (SUR).
