Authors' recommendations: There is not enough evidence stating that the MitraClip® percutaneous mitral valve repair system is better, or at least equivalent to conventional surgery. Low quality evidence shows that it could achieve a reduction in the severity of valve insufficiency and improve the functional class with mortality rates similar to those of conventional surgery at long term, but with higher rates or early re-intervention. The clinical practice guidelines and expert consensuses do not consider the use of MitraClip® as first choice in valve repair, but only as an alternative to be considered for selected patients (subjects with severe mitral insufficiency and at high surgical risk in the presence of comorbidities which do not shorten life-expectancy.
