[Transcatheter mitral valve-in-valve implantation (TMViVI)]

Authors' objectives: The transcatheter mitral valve-in-valve implantation (TMViVI) is a technology that is primarily used for elderly people with a failing mitral prosthesis. In addition, these patients usually have several co-morbidities. The valve-invalve concept has been so far primarily used for aortic valve replacement. However, since 2009 a small number of case reports has been published and there are a few, small-sized case series reporting about valve-in-valve applications for the replacement of dysfunctioning mitral valves. In Europe, the products of Edwards Lifesciences, Inc. (USA) are availbale for TMViVI. The aim of this systematic review is to evaluate the efficacy and safety of TMViVI compared to the standard surgical therapy, or no intervention
Authors' recommendations: The health technology is at an early stage of research. The results of 83 patients have so far been published worldwide. However, there is a research need for controlled trials. Patient-relevant benefits cannot be assessed adequately on the basis of the available studies. Currently, the inclusion into the hospital benefit catalogue is not recommended. A re-evaluation is proposed in 2017.
