Authors' recommendations: Aneurysms of the intracranial circulation are serious and potentially life-threatening disorders. An aneurysm is a weakened area in the wall of an artery caused by congenital defects in the vascular wall combined with degenerative changes that cause bulging, which can lead to rupture. Rupture of an intracranial aneurysm beneath the arachnoid membrane leads to bleeding into the brain or subarachnoid hemorrhage (SAH). SAH is an important cause of stroke mortality and morbidity. Each year up to 30,000 individuals are diagnosed with SAH, representing approximately 3% of all strokes in the United States. More than 5% of stroke deaths are due to aneurysmal hemorrhage, and more than 50% of these patients die within the first 30 days after the event. Survivors are often left with permanent neurological deficits. The incidence of unruptured intracranial aneurysms in the U.S. adult population is approximately 6%. For some aneurysms that have not ruptured (e.g., very small), specific treatment may not be needed, beyond monitoring of the aneurysm and associated symptoms, and treatment of comorbid medical problems and risk factors. The ultimate goal of intracranial aneurysm treatment is the permanent exclusion of the aneurysm neck and dome from the rest of the brain's circulation to reduce the risk of rupture. The treatment of intracranial aneurysms may be surgical, most often excluding the aneurysm from the parent artery by clipping, or by endovascular therapy with detachable coil embolization with or without the use of a supporting self-expanding microstent and/or the use of remodeling balloon angioplasty.
