Authors' recommendations: Total knee arthroplasty (TKA) is a commonly performed procedure for severe osteoarthritis. In the United States in 2012, there were 610,409 hospital discharges following TKA. Due to extensive dissection of soft tissues, bone cutting, and long operating times, TKA is associated with substantial blood loss, which may lead to acute anemia. Therefore, approximately half of TKA patients receive allogeneic blood transfusions (ABT) to prevent postoperative anemia. Although carefully screened, blood from a volunteer donor carries the risk of transmission of disease, and noninfectious transfusion reactions. Furthermore, the demand for allogeneic blood is increasing while the blood donor population is decreasing, resulting in an increase in cost and the threat of future blood supply shortages. As a result, research has been conducted into other blood-saving measures to reduce blood loss perioperatively, and to reduce the need for ABT.
