[Standards for the appropriate use of health technologies: diagnostic colonoscopy ]

Direct evidence from observational studies and indirect evidence from randomized controlled trials exists about the efficacy of screening colonoscopy for reduction of colorectal cancer mortality in standard risk asymptomatic population older than 50 years. There is evidence about the efficacy of colonoscopy for screening in moderate and high colorectal cancer risk population, for diagnosis of inflammatory bowel disease and for evaluation of rectal bleeding in people above 40 years old. The body of evidence is lower in other clinical situations such as surveillance after curative surgery for colorectal neoplasm, colorectal cancer screening in patients with inflammatory bowel disease or diagnosis of functional bowel disorders. We did not identified any systematic review or health technology assessment report about efficacy or safety of colonoscopy for post-polypectomy surveillance or evaluation of iron-deficiency anemia, which are two frequent indications for colonoscopy in our setting.
Colonoscopy is considered as a safe procedure. However, data about incidence of adverse events come from studies with a moderate-high risk of bias. It is necessary to perform more studies in order to better estimate frequency and identify risk factors for colonoscopic complications.
