Authors' objectives: The HOBIC program's third annual report focuses on measures in acute care and home care settings in Ontario. The report is divided into six sections: hospital coverage, assessment completeness, score changes, declines in activities of daily living, therapeutic self-care and home care. Comparisons are provided, where possible, between small and large participating hospitals. This information provides feedback to health care providers and policy makers to support quality improvement in health care delivery.
