Ontario stroke evaluation report 2013: spotlight on secondary stroke prevention and care

Significant and steady improvements in acute care outcomes and positive trends in the rehabilitation sector are being seen. The decreases in hospital admissions alone represent reductions of 3,300 ED visits, 4,400 inpatient admissions and 22,000 inpatient days, with annual savings estimated to be $27 million. In addition, the significant reduction in mortality represents approximately 330 deaths avoided per year.
Given the complexity of making changes in health care, as well as the aging population, it is impressive that steady progress is being made year over year. The OSN and the Regional Stroke Networks are well positioned to make further progress in those areas of the patient journey that have not shown improvement, most notably community care. There remains tremendous opportunity for improvement by addressing the gaps in stroke prevention clinic care and reducing LHIN variation. The OSN needs to continue to take a leadership role in collaboration with the Ministry of Health and Long-Term Care and Health Quality Ontario on the Stroke – Quality Based Procedures funding initiative to support implementation and evaluation for the inpatient acute care and rehabilitation sectors while developing a robust funding model that extends best
practices to community-based prevention, rehabilitation and care.
