Immunoglobulin gene testing in b-cell lymphoma

Lymphoma refers to a group of cancers of the lymphatic system, which includes lymph nodes, blood, and bone marrow. The World Health Organization (WHO) has classified > 40 types of lymphoma, with the 2 most common types being non-Hodgkin's lymphoma (NHL) and Hodgkin's lymphoma (HL). In 2013, > 700,000 people were classified as affected with lymphoma or in remission. Of these, approximately 170,000 and 550,000 have HL and NHL, respectively, and median age at diagnosis of 64 years. Approximately 85% of NHL cases are B-cell lymphomas (BCL), which are further categorized based on clinical symptoms and histologic and morphologic criteria. However, in 5% to 15% of patients, these methods do not provide a definitive diagnosis. In these patients, evaluation of immunoglobulin (IG) genes may provide further information to aid in diagnosis. These genes are involved in B-cell development whereby a series of rearrangements occurs in IG genes, resulting in a large number of products of multiple sizes (referred to as polyclonality). In cancer, however, an error during recombination may result in a single
rearrangement occurring multiple times, leading to the amplification of a single clone (referred to as clonality or monoclonality). Therefore, by analyzing specific regions of the IG genes immunoglobulin heavy locus (IGH) and immunoglobulin kappa locus (IGK), clonality or polyclonality may be evaluated to potentially provide information regarding malignancy, and provide supportive evidence for lymphoma diagnoses in combination with other criteria (which include clinical and histologic findings).
