Authors' recommendations: A large body of evidence from meta-analyses and randomized controlled trials indicate that compared with placebo, probiotics may reduce the incidence of antibiotic-associated diarrhea and may reduce the severity of symptoms related to irritable bowel syndrome. However, significant heterogeneity and low quality trials require conduct of further well-designed randomized trials that directly compare probiotics. Limited evidence from one meta-analysis suggests treatment with Saccharomyces boulardii in addition to vancomycin or metronidazole may reduce the incidence of recurrent C. difficile infection versus antibiotics alone.
