[Percutaneous transluminal septal myocardial ablation (PTSMA) in hypertrophic obstructive cardiomyopathy (HOCM)]

Authors' objectives:
Hypertrophic cardiomyopathy (HCM) is a rare primary myocardial disease with a prevalence of 0.2%. HCM is hereditary and frequently appears in familial clusters. In a portion of the patients (approx. 70%) it results, under exertion, to an obstruction of the left ventricular outflow tract: hypertrophic-obstructive cardiomyopathy/HOCM. HOCM is associated with sudden cardiac death. In addition to conventional methods (medicines), interventional or operative methods are also applied in the treatment of symptomatic HOCM patients.
Authors' recommendations:
The question whether PTMSA is equal or non-inferior to myectomy – in efficacy and safety – has been controversially discussed in forums for a long time. Three international professional societies have recommended myectomy as 1st line therapy. Since it concerns a very small, well-defined patient group and a volume increase is not to be expected, it is recommended that the intervention should only be carried out in highly-specialised centres with many years of experience in the interventional treatment of cardiomyopathies in Austria – upon the condition of participating in one of the already existing registries.
