Authors' objectives: Chronic Total Occlusions (CTOs) are defined as completely occluded coronary arteries with an occlusion duration of at least 3 months. This report compares efficacy and safety of percutaneous coronary interventions (PCI) to medical treatment or coronary bypass graft surgery (CABG).
Authors' recommendations: Due to methodological limitations of the included studies, the quality of evidence for safety according to GRADE is moderate. For efficacy, the quality of evidence cannot be assessed due to the absence of (prospective) controlled studies. The available evidence is currently insufficient to assess the efficacy and safety of PCI for CTOs in comparison to the respective standard therapies. Therefore, the inclusion into the hospital benefit catalogue is currently not recommended, either for operable or for inoperable patients.
