[Efficacy and cost-effectiveness of molecular techniques in the intraoperative assessment of the sentinel lymph node]

Authors' objectives: To assess the efficacy and the cost-effectiveness of OSNA intraoperative test compared to conventional techniques (intraoperative and postoperative) in the detection of metastasis in the sentinel node in breast cancer patients.
Authors' recommendations: The intraoperative OSNA assay presents a high agreement, sensitivity and specificity regard to the reference postoperative histopathological analysis. The intraoperative OSNA assay presents greater sensitivity than the conventional intraoperative tests of imprint cytology and staining of frozen sections. The OSNA clinical utility remains to be determined. Prospective studies would be required to determine their prognostic ability. With the limited information about economic efficiency available and the estimations performed, the OSNA test could be considered an alternative to histopathological analysis that would save costs, but it has lower diagnostic ability.
