Endovascular stent treatment of aortic aneurysms

Authors' objectives:

To assess the clinical efficacy and safety of endovascular stents in the treatment of aneurysms, at the request of the Direction Generale de sante (French Department of Health).

Authors' results and conclusions: The epidemiology, pathogenesis and treatment (surgery and stents) of aortic aneurysms are reviewed. Reference treatment is an open surgery with exposure and grafting of the aneurysm, requiring general anesthesia, laparotomy, aortic clamping and sometimes transfusions with a mortality of 3 to 5% for elective surgery and 50% in the case of rupture. Several models of endovascular stents are currently undergoing clinical evaluation. These studies are based on limited patient populations with a frequently short follow-up. The principal complications reported after the procedure are leaks or thrombosis of a branch of the stent. In the long term, periprosthetic leaks may be responsible for absence of thrombosis of the aneurysmal sac.
Authors' recommendations: None of the studies currently underway has yet demonstrated the efficacy and safety of endovascular stents in the treatment of aortic aneurysm. Consequently, evaluation studies must be pursued in two stages: study of the biomechanical feasibility and clinical value on a limited number of patients, followed by controlled, preferably randomized, studies with a relatively long follow-up (minimum of 2 years per patient) to evaluate the real efficacy and safety of these devices (especially in the prevention of rupture of the aneurysm). Other output or dissemination activity: To date, there is not yet in France coverage policy for these devices which are still considered as experimental.
