[Guidelines for chronic leg ulcers in primary health care]

Original Title: Manejo de la úlcera vascular de los miembros inferiores
Authors' objectives: Unify criteria of action and draw up therapeutic-preventive recommendations in the handling of leg vascular ulcers.
Authors' results and conclusions: 46% rate of responses from 305 consulted. 1117 ulcers recorded with an average duration of 30 weeks.50.4% were treated in primary health care clinic with 3.5 consultations/week; the rest were treated in house visits. Variability related with the type of leg ulcers is observed in treatment. 2487 hospital admissions, 78.5% for varicose problems. Average stay varies between 2.7 days for the varicose pathology to 23 days for the atherosclerotic disease.9% of these patients suffer amputations at different levels of the lower extremity. Any unified protocol has been used in our health care system.
Authors' recommendations: A Clinical Practice Guideline has been draw up which, besides indicating how to handle this pathology, recommends: 1) Diagnosis based on clinical record and on a complete physical examination, assessing the risk factors, 2) Establish the differential diagnosis of the vascular pathology, ruling out arterial participation in all venous ulcers, 3) Establishment of dietary-hygienic recommendations, 4) Standardisation of criteria of action and 5) Establishment of daily activity registers to achieve optimum attendance to patients.
Authors' methods: Bibliographic search of Medline, Escerpta Medica, Index Medicus and Index Nursing databases between from 1985 and 1995. Survey carried out among Primary Health Care nursing professionals over one week in June 1995. Study of hospital admissions by MBDS in 1994. Synthesis of the scientific evidence and preparation of the Clinical Practice Guideline.
