[Invasive telemonitoring in patients with heart failure]

Authors' objectives: Telemonitoring systems allow the measurement of cardiac pressures by a sensor located in the heart or pulmonary artery, so that they can adjust the pharmacological treatment of heart failure using objective physiological data obtained from the patient's home. To evaluate the effectiveness and safety of these devices a systematic review was made obtaining 11 articles, 3 of whom were high-quality clinical trials and 2 non-controlled experimental studies (beforeafter).
Authors' recommendations: The 4 systems tested (CardioMEMSR, ChronicleR, ImPressureR and HeartPODR) were described as safe and decreased the number of admissions and / or emergency rooms visits. There was only one clinical trial, conducted with CardioMEMSR, to demonstrate this significantly. CardioMEMSR demostrated a reduction of the risk of hospitalization related to heart failure in the intervention group versus the control group of 30% (0.31 vs. 0.44, p <0.0001) at 6 month follow-up. In addition, improved survival with less chance of hospitalization or death in the intervention group versus control (HR = 0.71, 95% CI 0.55 to 0.92) during follow-up period, and the quality of life patients too. CardioMEMSR made an economic study that increasing the ratio of integrating the system into the standard of care for the management of heart failure was estimated at $ 13,979 per QALY gained. The development of these systems of telemonitoring are an alternative to guide the pharmacological treatment of heart failure and their promising results must be evaluated over time.
