Authors' recommendations: Patients with severe coronary artery disease or impaired left ventricular function may be poor candidates for major cardiovascular surgery due to comorbidities, advanced age, or renal impairment. Although coronary artery bypass grafting (CABG) remains the standard of care for coronary revascularization, percutaneous coronary intervention (PCI) is increasingly viewed as an alternative. Patients with hemodynamic compromise at the time of surgery require support to perfuse the heart and vital organs during PCI. Stable patients may also require additional assistance due to the stresses imposed by injection of contrast dye, balloon inflation of stenotic arteries, and coronary stent manipulation. Hemodynamic support allows the surgeon to safety complete necessary procedures during PCI, restoring coronary perfusion. Placement of an intraaortic balloon pump (IABP) is the standard procedure for hemodynamic support.
