Authors' recommendations: Each year in the United States, approximately 37 million people develop chronic sinusitis. About one-third of these people see a physician for treatment of symptoms such as congestion, facial discomfort, headache, and general discomfort. Since there are many potential causes for chronic sinusitis, treatments vary and may include antibiotics, steroids, decongestants, and antihistamines. Adenoidectomy is the standard treatment for children with chronic sinusitis that is refractory to medical treatment, and is effective in 50% to 70% of cases. When combined with antral lavage, the success rate is about 80%. Functional endoscopic sinus surgery (FESS) is an accepted procedure for chronic sinusitis refractory to medical treatment. FESS is a minimally invasive technique in which the sinus air cells and ostia are opened and drained under direct visualization. Polyps and infected tissue can be removed at the same time. FESS is an accepted procedure for children with recurrent symptoms after adenoidectomy. While treatment success rates range from 80% to 100%, FESS can pose technical difficulties in children.
