[Paediatric triage systems in hospital emergency department: reliability and validity]

Authors' recommendations: Reliability: ED paediatric triage systems: MTS, PedCTAS, ESI, SET; SBRT showed a good or very good reliability. By contrast the ATS triage showed a low reliability. Validity: The validity of the ATS has not been assessed in ED pediatric population. The MTS, the only triage assessed against a reference standard, presented a moderate validity in pediatric triage with a high over-triaje rate (54%) and showed serious difficulties in the triage of children <3 months. In MTS, ESI, PedCTAS, SRTS and MAT SET triage systems, the triage level has proven to be generally a good predictor of hospital admissions rate, resource consumption and length of stay in ED. The 5-level triage systems seem to have good reliability and the triage level seems to be a good predictor of hospital admissions rate and resource consumption, although the available evidence do not allow to determine their external validity nor make an objective assessment of the benefit of pediatric triage programs versus no triage.
