The introduction of laparoscopic cholecystectomy in Quebec: effects on intervention rates and resource utilization - nonsystematic review

Authors' objectives:

To document the changes in rates of cholecystectomy associated with the introduction of laparoscopic techniques and to estimate their effects on direct costs to the Quebec health care system over the period 1984-85 to 1992-93.

Authors' results and conclusions: Prior to the introduction of laparoscopic cholecystectomy in 1989-90, cholecystectomy rates were falling in. In 1989-90 age adjusted rates were 349.5 and 166.5 per 100,000 women and men respectively, aged 10 and over. However, since that year they have increased steadily. By 1992-93, when 62% cholecystectomies were carried out via laparoscopy, there had been an overall increase in the number of cholecystectomies of 24%. The age adjusted rates then reached 413.8 and 195.6 per 100,000 women and men respectively. The accompanying increase in direct costs to the Quebec health care system is estimated to be approximately $8,09 million (+16.3%).
Authors' recommendations: The study showed that the introduction of an effective technology with a lower unit cost is, in fact, causing an increased financial burden. It is important that such increases be accompanied by demonstration of increased health benefits.
