Intraoperative Radiation Therapy (IORT) for breast cancer

Authors' recommendations: About 13% of women in the United States will develop breast cancer in their lifetimes with close to 40,000 deaths per year. Breast-conserving surgery followed by postoperative whole-breast external beam radiation therapy (EBRT) is the usual care for patients with early breast cancer and has shown to be a safe and effective treatment. Approximately 80% of recurrences after breast irradiation occur in the area of the original tumor site, leading to the emergence of radiation therapy techniques targeting this area. Intraoperative radiation therapy (IORT) involves radiation of the tumor bed while shielding adjacent radiation-sensitive organs and structures. It may be given as standalone radiation therapy following breast-conserving surgery (avoiding the conventional 5 to 6 weeks of postoperative EBRT) or as a boost to standard EBRT. For the former application, IORT is possible either as electron radiation therapy or as low-energy X-ray radiation therapy.
