[Surgical stabilization of a flail chest wall]

Authors' objectives: Is surgical fixation of rib fractures using titanium plates/intramedullary splint in patients with multiple rib fractures causing flail chest and impaired ventilatory function better than conservative non-surgical ventilator treatment regarding ventilatory function, pain, need for mechanical ventilation and health related quality of life (HRQL)?
Authors' recommendations: Surgical fixation of multiple rib fractures, as compared to conservative ventilatory support, in patients with flail chest and impaired ventilatory function improves ventilatory function after two months, decreases thoracic pain after one year and shortens the duration of mechanical ventilatory support (low level of evidence). The technique carries moderate risks mainly from wound infections and infrequent cases with non-union of the rib fractures. Organisational effects may include increasing number of patients admitted from other hospitals in the health care region to the Sahlgrenska University Hospital. Economical effects are difficult to estimate with current knowledge.
