[Economic evaluation of transanal endoscopic microsurgery]

Authors' objectives:

Transanal Endoscopic Microsurgery (TEM) equipment is used in the management of rectal adenomas and carcinomas. It has a high price and needs surgeons training (learning curve).Therefore its diffusion by now is slow, deciding on alternative techniques as local excision (LE) and radical surgery (RS), according to the extension and height of the tumour. Our aim is to undertake a cost analysis and budget impact study in the Community of Madrid.

Authors' recommendations: Our results are consistent with previous studies from U.S.A, Australia and U.K., where the use of TEM is able to save resources. It is recommended as elective technique in large rectal adenomas and malignant rectal tumours in stages pT1. In addition other recent publications propose its usage in other stage of the disease, which would affect the amortization of the equipment. However, the current study has signi‚cant limitations based on the study design and limited available evidence. Further research is required to replicate these results.
