[Surgery guided by imaging assessment: efficacy, safety and economic impact of Intraoperative Magnetic Resonance Imaging]

Authors' objectives:
To assess the efficacy, safety and economic assessment of Open Magnetic Resonance Imaging (open MRI) and intraoperative. This technology allows to make interventional/intraoperative procedures, like surgery of the epilepsy, brain tumour resection, biopsies, electrodes insertion. Useful in claustrophobic patients, large persons and young patients since they do not need anesthesia. The scanners normally had low magnetic field (0,2 T) and high magnetic field strength (1,5 T).
Authors' recommendations:
Evidence level is low due to the studies design, mainly non comparative studies. There is some information related to gliomas surgery from two comparative cases series studies. In one there is not statistically significant difference with the new technology in comparison with the conventional surgery. In the other study the most optimal results were obtainedwith the combination of MRI of high field with neuronavigation system, obtaining smaller residual volume. The follow-up is not well defined in some patients with residual tumor and also when there is not histopathological confirmation. The quality of the interventional images with 0.2 T system substantially improves when combining these images with the preinterventional images obtained with high field 1.5 T system in the electrodes insertion during neurosurgical procedures although is a single study results.This technique is beginning to use in other pathologies as hepatic or renal tumors.
