[Standards for appropriate use of health technologies. Total hip arthroplasty]

Authors' recommendations: Factors most frequently studied in relation to their influence on total hip replacement surgery outcomes are age, gender and body mass index. Other factors assessed in some of the included studies are comorbidities and presurgical degree of functional limitations and pain. Most of the included studies were observational, corresponding to a limited level of evidence. In addition, the relationship among patient dependent factors and the different surgical outcomes has been found to be complex. For these reasons, clinical judgment of experts is fundamental in order to make an appropriate assessment of risks and benefits of the intervention at the individual level. Given that the effectiveness of total hip arthroplasty is clearly established, appropriateness of patient selection for the intervention is of critical importance, in order to facilitate the access to the surgical procedure to all those patients with a favorable risk benefit ratio and to avoid the unnecessary exposure to surgical risks to patients that can expect more risks than benefits from the procedure.
