[Using a knee brace after reconstruction of the anterior cruciate ligament]

Authors' objectives: To determine the efficacy, effectiveness, safety and cost of knee braces for the control of knee joint stability after ACL reconstruction.
Authors' recommendations: Studies evaluating the use of knee braces in the rehabilitation period after ACL reconstruction are few, not recent and of low methodological quality as experimental studies. The study characteristics and measured outcome variables are very wide-ranging. This meant a meta-analysis was not possible. Available studies do not show better results for patients using knee braces, when compared with those who do not, for knee laxity, various functional tests, the range of movement or pain reduction. Therefore the evidence available does not support the routine use of a knee brace in the rehabilitation period after ACL reconstruction (Grade B recommendation). Clinical experience suggests that these braces may be of some benefit in certain cases (eg, patients who do not comply with postoperative rest advice, patients with a greater risk of re-rupture due to practising certain sports, or when there are technical problems during surgery). Therefore, their use in this type of patient or in these circumstances should be carried out using a suitable research protocol to properly evaluate their contribution.
