[Integrated intra and extra-hospital approach to conduct disorders in adolescence]

Original Title: Abordaje integrado intra y extrahospitalario de los trastornos de conducta en la adolescencia
Authors' objectives: 1. Determine the demand for health care directly relating to CD among adolescents in the public health network of the Basque Country. 2. Analyse the techniques and organisational structures deployed in the Mental Health network of Osakidetza-Basque health service and in other national and international health systems to deal with such disorders. 3. Review the national and international literature on this subject. 4. Prepare proposals on the care of CD in adolescents with a view to their possible implementation in the Basque Community in order to improve current levels of care.
Authors' results and conclusions: There are no epidemiological studies of the prevalence of Conduct Disorders in adolescents in our environment. The data from this research provide the initial evidence and reflect the high frequency rates: they are the motive for consultation of 25-40% of the adolescents who receive care at outpatient mental health units and are the cause for the admission to psychiatric units of children-young adults. There are no specific treatment programmes comprising intra and extra-hospital care for adolescents with CD in the Basque Country or at a national level. Therapeutic approaches that have demonstrated their effectiveness according to a number of different international research studies are contingency management conduct programmes, cognitive models of training in problem solving skills and in social skills (it is recommended that these be carried out in group environments), training for parents in the conduct management of their children, functional family therapy and multi-systemic therapy. We present a specialised therapeutic proposal for treating CD in adolescence, which would replace the pressing need for care, as well as representing an innovative project at a national level of feasible implementation.
Authors' recommendations: The Intensive Treatment Programme for Adolescents with Conduct Disorders that we propose includes a coordinated approach both at an intra and extra-hospital level. It comprises a number of treatment models designed according to the needs of each case. It includes therapeutic approaches the effectiveness of which has been demonstrated. It consists of the following modules: (1) complete psychiatric hospitalisation module, (2) partial psychiatric hospitalisation module, (3) individual treatment module (psychotherapeutic and psychopharmacological), (4) group training module in psychosocial skills, (5) working with parents module and (6) systemic family psychotherapy module.
Authors' methods: Analysis of health records available in the Mental Health network of the Basque Country together with a more detailed examination of the data from each centre in which researchers are working in order to determine the current demand for care of CD in adolescence. Gathering of data on the real care practices offered to patients with CD in our environment. Gathering of data on organisational structures in this field, both in our environment at a national level and in systems of recognised prestige abroad. Systematic review of the scientific literature on the care provided for Conduct Disorders in adolescence and organisational models. Analysis of the data obtained, discussion and drafting of a report with specific recommendations.
