Authors' objectives:
This report evaluates the usefulness of PrometheusTM in patients with severe liver failure.
Authors' recommendations:
Not many published studies have been found which use PrometheusTM. The number of patients included is scarce, the medium and long-term follow up is little and the reported results are only interim. In the assessed studies, only two have evaluated the use of PrometheusTM versus standard care and no improvement has been reported in terms of survival, except for one of them which showed benefits for those patients with hepatorenal syndrome I or with a MELD score >30. Also, most of the studies compare it with MARSTM showing similar results in most of the evaluated parameters measured in terms of substance clearance. However, MARSTM showed better results in mean blood pressure. No conclusions may be drawn as to survival or other relevant results such as liver encephalopathy.Although it seems to be a technology with correct adverse effects and safety parameters, and with a performance similar to MARSTM, the available evidence is poor.More studies with more patients, longer follow ups and more relevant results are required to be able to draw definite conclusions.
