Wound care represents a substantial healthcare challenge, especially for chronic, nonhealing wounds. To facilitate healing, dressings are applied to protect the wound from contamination, to maintain a moist wound bed, and to initiate tissue replacement. Therapeutic honey wound dressings have been used since ancient times. Therapeutic honeys are highly viscous honeys that are sterilized using gamma irradiation to destroy bacterial spores while retaining biological activity. Honey is a supersaturated sugar solution with acidic pH that has a range of therapeutic effects. It has been shown to reduce both acute and chronic inflammation and to stimulate the body's immune system. The osmotic effect of honey reduces edema and alleviates pain. Honey is also a broadspectrum antibiotic and has low potential for bacterial resistance. Honey may also facilitate wound debridement and deodorize wounds. Most research has been done on active manuka honey derived from Leptospermum sp (tea trees) native to Australia and New Zealand. Honey can be applied directly to the wound bed, or on gauze dressing and placed over the wound, or a commercially available product can be used.
