Authors' objectives:

The primary reason for surgery of discal prolapse is to alleviate the irritation or compression of nervous root that occur due to herniated disc. The support to alternative surgical procedures has been given under absence of enough evidence based on clinical outcomes. The objective of this review was to evaluate the effectiveness of nucleoplasty in the treatment of discal hernia.

Authors' recommendations: Chemonucleolysis nucleoplasty is more effective and less invasive than placebo. On the other hand, it is less effective than surgical diskectomy. Patients carefully selected with sciatic pain due to disc prolapse seem to have more improvements in their acute crisis than in the conservative treatment. However, any positive or negative effect in the natural history of this disease is not clear.
