BRATS 07: Deferasirox for the treatment of iron overload

Authors' objectives:

The topic of this bulletin is a drug which was recently incorporated into the Unified Health System, that is, deferasirox. Administered orally, this drug is indicated for the treatment of chronic iron overload, due to blood transfusions, in adult and pediatric patients.
Desferoxamin meets a larger number of indications and is associated with a survival period of approximately 50 years; however, adherence to the treatment is low due to pain and subcutaneous infusion intolerance. Although administered orally, deferipron’s use is limited due its adverse reactions profile.

Authors' recommendations: Due to the low prevalence, there is little evidence on prognostic implications of iron overload in patients with falciform disease or mielodysplasia syndrome, as well as on benefits of iron chelators in such patients. Besides, recurrent transfusions may not be necessary in patients with falciform anaemia and patients with mielodysplasia syndrome normally elders and may not survive to accumulate iron bordering with the overload. As such, implications related to iron overload in patients with falciform disease and mielodysplasia syndrome may not have the same consequences as the ones observed in patients with beta thalassaemia.
