La coordinación entre atención primaria y especializada desde la perspectiva de los profesionales
Authors' objectives:
The aim of this research is the development of a deep approach to the coordination between Primary and Specialised Healthcare from the point of view of the professionals who develop it, right from the identification of values, beliefs, attitudes, habits… that determine their behaviour in the healthcare process. Secondly, we should try and help improve the coordination among the different stages, identifying some improvement areas and finally, we should set some priorities in the detected lines of work.
Authors' results and conclusions:
Most of the ideas gathered have been repeated both in the bibliographical research as well as in the interviews and focus groups. They are put forward as follows:
- The most relevant improvement areas which have been detected belong to the field of communication.
- The professionals claim that with the help of the shared medical history an important improvement in the field of coordination will be achieved.
- The human factor is fundamental in the coordination.
- Promote mutual understanding among the professionals in both groups would result in a considerable improvement in the coordination.
- We have not been able to go as much into the values and attitudes in depth as we had expected.
- In most of the discussions which took place in the focus groups and interviews a lack of reference to the patients has been detected.
- The feeling that both the GPs and managers have regarding the coordination between Primary and Specialised healthcare is not as different as expected.
Authors' recommendations:
The lines of work that we recommend should be put first are as follows,
- Development of a shared medical history.
- Development of a forum between Primary and Specialised Healthcare so as to encourage the mutual understanding of the professionals, the communication and development and set in motion of joint strategies such as agreed aims between Primary and Specialised Healthcare, clinical sessions and shared training processes, improvement in the referral note and method in the nursery care, establishment of guides for clinical practice, development of referral protocol, among them.
- To encourage the use of telemedicine in order to improve the efficiency in the medical assistance in some processes and specialities on which we focussed.
Authors' methods:
The method used was divided into three stages. In the first stage, a revision on the literature and on the documents was carried out. In the second one, five interviews were carried out, as well as three focus groups in order to watch and analyse the reality of the phenomenon. In the third and last one, priority was given to the lines of work extracted and gathered during the second stage and we proceeded to draw the conclusions and recommendations of this research.
