[Validation of an assessment scale of the risk of pressure ulcers in hospitalized children]

Original Title: Validación de una escala de valoración del riesgo de úlceras por presión en niños hospitalizados
Authors' objectives: The aim of this study is to translate and validate a valuation scale of the risk of developing pressure ulcer (PU) in hospitalised children: the Braden Q scale. The specific aims are as follows: - Determine the validity of the contents of the Braden Q scale in its Spanish version. - Establish the internal consistency of this version of the scale. - Determine the intra-observer and inter-observer agreement of the scale.
Authors' results and conclusions: 738 patients were studied, of which 60.4% were girls and 39.6% were boys. 87.3% were non-critical patients and 12.7% were critical. The exploratory factorial analysis revealed two factors: one containd the «mobility», «sensorial perception», «humidity», «shear friction» and «tissue perfusion and oxygenation» variables and the other, the «activity» and «nutrition» variables. The scale showed good internal consistency with a Cronbach alpha of 0.695. In the reliability study of the scale, the intra-class, inter-observer correlation coefficient for children of less than one month was 0.566 (IC 95% 0.454-0.659) with Kappa agreement indices of under 0.5 in all subscales except in «nutrition». The Kappa interobserver indices in children of 1 month to 2 years is above 0.6 in all these subscales with the exception of «sensorial perception» and «tissue perfusion and oxygenation». In the groups of 2-6 year olds and 6-14 year olds, these are high in all the subscales, with values of over 0.7 in most of them. There are no differences in reliability according to age groups in the critical/non-critical subgroup analysis. In the analysis of inter-observer and intra-observer reliability in the group of patients of over two years of age, the inter-class correlation coefficients are 0.943 (IC 95% 0.930-0.954) and 0.975 (IC 95% 0.969-0.980) respectively, with Kappa indices of over 0.7 in the analysis of agreement according to subscales.
Authors' recommendations: The Braden Q scale has been shown to be a valid and reliable instrument to assess the risk of developing PU in hospitalised children of from 2 to 14 years of age, both critical and non-critical. Nevertheless, it is not a reliable scale in children aged less than 31 days. In children of 1 month to 2 years of age, the overall reliability is good. However, it does not reach acceptable agreement levels at an inter-observer level in some subscales. The need to carry out small-scale studies in this age group is proposed.
Authors' methods: Multi-centre observational survey in 13 paediatric hospitalisation units of 4 hospitals of the Osakidetza–Basque Health Service network. The study included hospitalised children of between 0 and 14 years of age. Translation/back-translation method of the original scale into Spanish. Study of the psychometric properties of the scale: analysis of the validity of the contents, internal consistency and intra- and interobserver agreement.
