[Examination of barriers and facilitators for the implementation of clinical practice guidelines: a Delphi study]

Original Title: Exploración de barreras y facilitadores para la implementación de guías de práctica clínica: un estudio Delphi
Authors' objectives: To determine the relevance of the different personal or external factors, facilitators or barriers, that have an influence on the acceptance of two CPGs (asthma and High Blood Pressure - HBP) in our environment.
Authors' results and conclusions: 100% of experts considered that the use of CPGs has a favourable influence on the health of patients. The results of the different aspects are as follows: 1. Dimensions of the CPG: The presentation of the CPG in health centres is considered to be the main dimension, the adequate dissemination of the announcement, the didactic method and the choice of presenter being of special interest. 2. Internal barriers: The resistance to change among doctors and a lack of acceptance in their use as a tool are the most important of these. 3. External barriers: The lack of agreement between Primary Care and Specialised Care is an essential aspect according to experts.
Authors' recommendations: Although most professionals consider that by following the recommendations set out in the guidelines results good can be obtained in clinical practice, the former presentation, the current format of the CPGs, the resistance to change among health professionals themselves and the low level of involvement of specialised care professionals represent major barriers to the successful implementation of CPGs.The results of this study may be useful in the design of future implementation strategies aimed at overcoming the barriers in our environment.
Authors' methods: The two-round Delphi technique was used in which, based on a questionnaire prepared from the results of a review of the literature and of information contributed by the coordinator group, the group of experts (36 primary health care doctors) pointed out those elements they considered to be the most important barriers and facilitators in the implementation of CPGs. The questionnaires were drawn up to assess a number of different aspects: Form of presentation, format, ease of use and usefulness of CPGs, internal barriers and external barriers. The analysis was made through consensus (relative interquartile range and variation coefficient) and stability criteria (relative interquartile range variation and variation coefficient variation).
