Authors' recommendations: Based on the current level of evidence, the relative safety and efficacy of IORT in comparison with breast conserving surgery with postoperative radiotherapy is still uncertain. However, early IORT results suggest only minor complications in the short-term and the cosmetic outcome from IORT appears to be as good as breast conserving surgery with postoperative radiotherapy. More rigorous studies with adequate sample sizes and long-term follow-up periods are required to assess the long-term rate of radiotherapy complications, local recurrence rates and disease-free or overall survival. In addition to this, studies on the cost-effectiveness of IORT will also need to be determined before IORT can be applied in routine clinical practice.
